Billing Medi-Cal as a doula group

Starting a doula collective or nonprofit and billing Medi-Cal under a group NPI? The claims themselves are the easy part. The hard part is keeping track of which doulas are enrolled with which plans, and billing everyone’s visits under the group. Here’s how one person can do it for the whole team.

Updated October 26, 2026

  1. The model: the group bills, each doula renders

    A doula group bills Medi-Cal as an organization. The billing provider on every claim is the group: its organization (Type 2) NPI, tax ID and address, and Medi-Cal or the plan pays the group. The rendering provider is the doula who did the visit, with their own individual (Type 1) NPI.

    In PracticeLite, open Insurance → Setup and set the billing profile to As an organization, with the group’s NPI, tax ID and taxonomy code (374J00000X for doulas). Then add each doula’s NPI on their provider card.

    The group’s name, NPI and tax ID on claims must match its Medi-Cal enrollment exactly. A mismatch is the most common first rejection.

  2. Track enrollment per doula and per plan

    Enrollment is the slow part, and it happens on two levels. The group contracts with each Medi-Cal managed care plan it bills (and enrolls with Medi-Cal itself). Each doula enrolls with Medi-Cal and is credentialed with every plan, under the group.

    On Insurance → Setup, the Group enrollment card records the group’s status with each plan, the plan’s group or contract number and the effective date. Its Who’s enrolled where grid shows the group and every doula against every plan: approved, waiting on the payer, in progress or not started. Each doula’s card keeps their own checklist for each plan.

    A claim to a plan the group is tracking checks that the group is approved before it can go out, and every claim checks the rendering doula’s own enrollment with that payer.

  3. Doulas log their own visits

    Add each doula as a practitioner with their own login. They see their own schedule and clients, book and log visits, and write their notes. That’s their whole part in billing: every past visit with an insured client becomes something the biller can claim.

  4. The biller turns the team’s visits into claims

    Give the person who does billing the Biller role (or Admin). In Insurance → Claims, the Not billed yet queue lists every past visit, by any doula, that isn’t on a claim. Bill a client’s visits and the claim fills in:

    • the doula who did the visits as the rendering provider (one doula per claim, so each is credited correctly);
    • the doula codes from your appointment types (Z1032, Z1034, Z1038, 59409 and the rest);
    • the XP modifier Medi-Cal requires on every doula line, and 95 for a video visit;
    • the client’s diagnosis codes, picking ones Medi-Cal allows for each code.

    Before it goes out, the checklist applies Medi-Cal’s limits across the whole team, using every claim for that client, whichever doula made the visit: eight prenatal visits per pregnancy, one doula visit a day, labor and delivery once per pregnancy, and the rest.

  5. Submit, then follow it to paid

    Press Submit claim and it goes to Medi-Cal or the plan electronically, through PracticeLite’s clearinghouse, with no separate account. The Claims list sorts everything into queues: needs action, waiting on the payer, paid and denied.

    Payments post to the claim from the remittance. A denied claim shows the payer’s reason and your options: Send a corrected claim (a fixed copy that replaces the original at the payer), start an appeal, or close it.

  6. What it costs

    Groups are on a program plan priced by how many clients you serve in a month, not by how many doulas you have. Insurance billing is included, and PracticeLite never takes a percentage of what you collect. See program pricing, or book a call and we’ll size it with you.

Common questions

Does each doula in a group need their own NPI?
Yes. The group bills under its organization (Type 2) NPI, and each doula is the rendering provider under their own individual (Type 1) NPI. Both are free from NPPES.
Does the group or each doula contract with Medi-Cal plans?
Usually both: the group contracts with the plan, and each doula is enrolled with Medi-Cal and credentialed with the plan under the group. Plans differ, so check each plan’s doula process; our plan-by-plan guides cover each one.
Can one person do the billing for the whole group?
Yes. Doulas log their visits on their own logins; a biller (or admin) sees every unbilled visit in one queue, builds the claims with each doula as rendering provider, submits them and follows them to paid.
How is PracticeLite priced for a doula group?
On a program plan priced by how many clients you serve, not per doula. Insurance billing is included, and there is no percentage of collections.
